Our Approach to Supportive, Independent Living

The Grove at Cottonwood is designed to support independent living with the right level of connection and coordination—not to provide direct care services. Residents live in their own homes and maintain autonomy, while having access to a supportive, community-oriented environment that promotes stability, inclusion, and long-term success.

BHA will employ a full-time, on-site Resident Services Concierge (RSC) who serves as a community-facing, non-clinical team member. The RSC helps strengthen housing stability through connection, engagement, and resource navigation—supporting residents in building routines, accessing services, and participating in community life. This role complements—but does not replace—formal coordinated care, case management, or other Medicaid-funded services provided by outside partners. It is not a caregiving position and does not provide hands-on or medical care.

The Grove does not have 24/7 on-site staffing. Residents are expected to live independently or with the appropriate level of external support already in place. While not required for tenancy, many residents may choose to work with a Provider Agency through Colorado’s Home and Community-Based Services system. These agencies offer coordinated care, independent living supports, and access to 24/7 on-call assistance as needed.

We work in partnership with organizations like Friends of Broomfield and Imagine! to help coordinate opportunities for engagement and connection. This model is intentional—designed to ensure residents have access to the resources they need while preserving independence, choice, and dignity.

BHA connected with communities across the country to learn from established models supporting individuals with intellectual and developmental disabilities. Through these conversations, we identified best practices around balancing independence with meaningful support and community integration. These insights directly shaped the service approach at The Grove.

Resident Services Concierge – Key Responsibilities

  • Plans and coordinates inclusive community events and activities, supports resident-led programming, and helps create opportunities for connection both on-site and in the broader community

  • Serves as a resource connector, helping residents navigate community services and link to appropriate providers, supports, and programs based on individual goals and needs

  • Welcomes new residents, supports onboarding and orientation, and provides ongoing engagement to help residents build community, routines, and stability

  • Partners closely with property management and maintenance teams to support clear communication, address concerns early, and help maintain a stable and positive living environment for all residents


Centering Resident Choice

Residents retain full autonomy in selecting service providers that meet their needs involving coordinated care, clinical case management, day programming, employment services, behavioral services, etc. Participation with FRIENDS of Broomfield, Imagine!, or any other Provider Agency, is entirely voluntary and based on individual preference and eligibility. The RSC will coordinate with any service partner a resident chooses to include in their Circle of Support, reinforcing self-determination and person-centered planning.

Direct On-Site Programming Support

At this time, The Grove at Cottonwood is partnering directly with FRIENDS of Broomfield and Imagine! to support community life and successful independent living at The Grove by providing:

  • Referrals for the project-based vouchers and general property waitlist

  • Coordination with the RSC to support on-site programming and community-building efforts

  • Volunteers to support on-site social amenities and community engagement opportunities

  • Collaboration with the RSC to align resident goals and ensure needs are being met and supports and services are complementary and not duplicative

  • Medicaid-funded coordinated care and specialized services for eligible residents who choose to utilize their services
